Once again we would like to thank our local doctors and nurses for providing this opportunity for our MVHS and MVJHS student athletes. They, along with RPI, are the reason that we can provide this FREE service to students who will be participating in athletics at MVHS and MVJHS for the 2024 - 2025 school year. Incoming 9th through 12th grade: Pre-screening for off-season athletes will start at 5:00 PM. In-season athletes should arrive starting at 5:30 PM. No admittance after 6:30 PM. Online athletic registration for the 2024-2025 school year will open May 1 for all incoming high school athletes at mtvernon-in.finalforms.com. If your athlete has previously registered on FinalForms, their data has been saved and you will simply need to login and review/update their information. For those who have not previously registered, please go ahead do so! (Be sure to choose the 2024-2025 school year!) Once the registration is complete, you can print the IHSAA physical form directly from the last page of the FinalForms registration. (This saves you from having to complete the paper copy!!) The athlete and parent/guardian will both need to sign the printed copy at the bottom of the evaluation form and Page 4 MUST BE COMPLETED (including insurance information) and signed by parent/guardian and athlete prior to obtaining a physical. The student athlete should bring the entire physical form with them to the FREE physical night. If you have questions, call the Athletic Office at (812) 833-2060. ***Incoming 9th grade athletes will also be doing concussion screening. Athletes should bring their chromebooks with them to the physical to use for the screening. Incoming 7th and 8th grade: 6:30 PM. No admittance after 7:15 PM. The required Athletic Waiver can be downloaded from the Jr. High website (https://www.mvschool.org/jh-home) or picked up in the Jr. High Office. This form MUST BE COMPLETED by a parent/guardian prior to obtaining a free physical.
